Q. Who will need home health care?

A. Those needing short-term assistance after being discharged  from the hospital, skilled nursing facility or rehabilitation center.

- Chronically ill, terminally ill, or permanently disabled who will remain at home.
- Those who are ill or disabled, needing assistance at home.
- Those who need short term relief for family members caring for a parent relative who is ill.

Q. How do I pay for home health care?

A. If you are eligible for home health care, we will bill your insurance company directly.  We work with Medicare, Medicaid & most commercial insurance companies.

Q. What is hospice?

A. Hospice services are provided to assist patients and their families with end of life care. Hospice care allows the patient to remain in his or her own home surrounded by their family. Hospice care is as much for the patient as it is for the family. Hospice treats the person, not the disease.

Q. Who needs hospice services?

A. Hospice is for anyone who is not seeking a curative treatment for a terminal diagnosis, and they are nearing the end of life. End of life can be anywhere from one day to one year based on the disease process. Senior adults, adults and children may qualify for hospice care.

Q. What home health care and hospice services are offered by Aspire?

A. Aspire offers an array of services which include skilled nursing, physical, occupational & speech therapies and medical social work.

Q. How do I know what types of home health care and hospice services I need?

A. Any services and visits that you receive will depend on the home health assessment given by our staff and your doctor.  There will be a personalized plan of care that will be established to meet your needs.  This personalized plan will help determine the different services you may need and the frequency of home care visits.

Q. How do I set up home health care and hospice services?

A. You can call our intake department and speak with one of our trained staff, and they will guide you through the initial process.

Q. Will my health information be disclosed to anyone?

A. All of your information remains confidential with the agency.  The agency will not release any information unless authorized by you or your legal guardian/representative.

All the Indian Territory [Aspire] nurses on our Hospice Team, lead by Michelle, gave my husband exceptional care. They were a great comfort to both of us and were ever ready to bring whatever medication and medical supplies we needed. They had to brave the snowy, rainy winter weather and deplorable condition of our dirt roads. I commend them all on their professionalism, dedication and their quick responses to my emergency calls.
